SMART GOALS
A SMART Goal is a goal
which meets five criteria that are used to determine if the goal can be
achieved and how it will be achieved. The form below will help you determine if
your goal is a “SMART Goal.” These are the five criteria:
SPECIFIC:
What exactly do you want to accomplish? Be specific!
MEASURABLE:
How will you measure progress? How will you know that you have reached your
goal?
ATTAINABLE: Are you sure you can achieve your goal? What will you need to make it happen? Consider modifying your goal if it is not realistic.

RELEVANT: Is the goal really important to you? Why does it matter to you?
TIME BASED:
When will you start and finish this goal? Create a schedule of steps needed to
reach your goal.
